Application fees are charged (the site notes $50 for hospital affiliates and $15 for university affiliates).
Eligibility to apply is limited to current or incoming Stanford University or hospital affiliates in benefits-eligible roles, matriculated students, appointed postdoctoral scholars, and certain visiting scholars or faculty; active affiliation must be maintained to keep enrollment.
